Housing

THE TRANSITION HOME

The Transition Home provides safe and affordable housing with flexible supports in a homelike atmosphere. The home is staffed 24 hours a day. The program is for youth 16 to 24 years of age and includes the following services: subsidized supported housing, individualized case planning, individual and group counselling, life and social skills training, and assistance in preparing the young person to make the transition back home or to a more independent living situation. Referrals will also be made to other community resources as required.

Program Expectations
  • The home provides supportive housing for nine youth. Below are the guidelines that keep the home safe and provide a healthy environment for everyone participating in the program.
  • Smoking is permitted outside only
  • No drugs or alcohol
  • All residents are required to have a day program (school or work)
  • No weapons
  • You have to be home by 10 pm weeknights, 12 am weekends
Moving In

Admission to the Transition Home is through an assessment. When the resident moves in they are paired with a Residential Counsellor. The Counsellor assesses the resident’s life and social skills and the resident, throughout their stay, works on the life and social skills they require so they can move on to more independent living or return home.

In addition to the practical life skills offered, the Transition Home Team works closely with Youthab’s Mental Health Team. Recreational activities occur weekly and guest speakers are a common occurrence.

Upon completion of the program, Youthab assists the resident to move back home or into a more independent living environment. Youthab can provide several housing opportunities, depending on what is best for the resident.
